Requesting New Placement Application Forms or Edits
Placement Application Forms are agreements, forms, or required uploads that students must complete for each placement application.
New Placement Application Forms and edits to existing forms are set up in collaboration with your Account Manager. Form changes are typically completed within a few days of the request, but may take up to two weeks depending on demand.
You can submit your request in either of two ways:
- Email your Account Manager or help@supervisionassist.com
- Submit our Placement Application Form request form

Requesting New Placement Application Forms
Provide each form in one of the following formats:
- A link to an existing electronic form
- Form questions in a Word document
- A PDF of the form
If you use the request form, list in the comments field the name of each form you uploaded and which training levels must complete it. If you are sending an e-mail please use the e-mail template listed below.
For each form, include:
- Form Name: The title as it should appear in Supervision Assist.
- Submitter(s): Who completes and submits the form (e.g., Student/Trainee, Faculty Supervisor, Site Supervisor, Coordinator). If more than one person completes the form, list submitters in the order they complete it.
- Training Levels: Which training levels this form should be enabled for (e.g., Practicum, Internship).

- Submitter 1: Student/Trainee

Student Ethics Agreement
- Submitter(s): Student/Trainee
- Training Levels: Practicum, Internship
Requesting Edits to an Existing Form
Contact your Account Manager or help@supervisionassist.com with:
- The name of the form as it appears in Supervision Assist
- The specific changes you need
- When the changes should take effect
Note: Minor changes, such as grammar or typo fixes and removing a question, can usually be handled by editing the existing form. Larger changes, such as adding new questions, changing response options, or moving sections, are handled by creating an updated version of the form.
