Editing Activity Log Details / Change Placement

1. On the Activity Log page in Supervision Assist, click on the entry that you wish to edit (shown below).

2. After clicking on the entry, another screen will be brought up listing all of the details for that entry.

3. On the next screen you will be able to edit that entry's details as well as the options that are available to select. After you have made the necessary changes, click save on the bottom right of the page.


Changing Placements

Changing Placements with In-Progress Applications

If you have a placement application that has been submitted, but not yet been approved you may change your site supervisor by following the instructions below.

  1. From your dashboard click Placements then View All Placements.
  2. Select the desired placement application.
  3. If your application has not yet been approved, you may:
    1. Change your Supervisor
    2. Change your Supervision Site
    3. Change your Training Plan


Any Required Forms with the site supervisor as a submitter will be reset and must be completed again.


Changing Sites or Supervisors After Application Approval

If your Site or Site Supervisor has changed after you have already completed hours please follow this guide and create a new Placement Application to connect to your new site or supervisor.
